Blueprint was initially engaged to explore a strategic sale or lease of a 204-bed skilled nursing facility generating impressive revenues of $12.1M and EBITDAR of $1.2M, despite operating below market occupancy.
As the mandate evolved, the client opted to engage Blueprint for the sale of a second 122-bed skilled nursing facility.

Both facilities were strategically located in high density areas that were in close proximity to multiple large referral sources.

Blueprint positioned the offering as a rare opportunity for an incoming owner/operator to improve occupancy levels by capitalizing on high-acuity Medicare referrals, while also leveraging in-demand ancillary services such as in unit ventilators as well as bedside and outpatient dialysis.

Given its industry leading skilled nursing activity, Blueprint was able to execute on a highly confidential marketing process that resulted in seven initial offers.

While the Seller had initially moved forward with a regional owner/operator who had an existing presence in Arizona, after months of ongoing negotiations Blueprint, in conjunction with the Seller, pivoted to the Ensign Group.

By acquiring the real estate through its captive real estate subsidiary, Standard Bearer Healthcare REIT, Ensign was able to close on the transaction just 45 days after going under LOI.
