The two facilities are licensed for a combined 250 beds and located one hour apart. Built in 1997, Parkview Manor is located northeast of downtown Green Bay and is the largest facility by licensed capacity in the competitive market. Omro Care Center is situated in the smaller town of Omro near Oshkosh along the Fox River. Representing the operator's only locations in the state, the facilities struggled to generate positive cash flow.

Blueprint positioned the value-add offering as an opportunity for an incoming buyer to grow a regional platform realizing key cost center savings and operational synergies. Strategically marketing the offering to a targeted audience of owners and operators with an existing presence in the region, Blueprint identified an owner-operator prospect eager to expand its presence in Packer country. The sale was the final disposition executed by Blueprint as part of a larger portfolio optimization for the real estate owner and operator.
